We are honored that you have chosen us to be your primary care practice and we are committed to providing you with the best care possible. It is our goal to provide you with the highest level of patient care.  We welcome your feedback, encourage your health care participation and look forward to working with you towards your good health.

Due to high demand we are currently at full capacity for medical patients. At this time, Dr. Howlin will not be accepting any new medical patients. Thank you for your interest.

Cancellation Policy

Please notify us 24 hours in advance if you must cancel a medical appointment. That allows us to give the appointment to another patient who needs it. Because of our open access scheduling, it is important that our patients keep their scheduled appointment times. Any appointment that is cancelled in less than 24 hours or if any appointment is missed without notice, you will be charged our missed appointment fee of $35. This is not enforced in inclement weather. Your safety is of utmost importance to us. Drive safely!

On Call Schedule

We understand that no one plans for a medical emergency, so we provide 24 hour accessibility. If you need medical care after hours, please call our main office line (734) 475-2921 and follow the directions on how to contact your physician. We ask that you only use this service for emergencies (not to change an appointment or request a prescription refill).  In case of chest pain, high fever or acute injury you should go directly to the emergency room. Any non-emergency needs can be left on the voicemail and our staff will help as soon as possible. 

Medical Forms

Insurance forms, FMLA forms, disability forms, etc. we are happy to help with these necessities of life, but will charge $25 to complete due to the extra time it takes for the doctors and staff to research and complete them.  We ask that under normal circumstances you give us at least two weeks to complete and mail any such form.
