Payment Options & Insurance

Primary Care

We accept most major insurance plans. Co-pays are due at the time of your appointment. Any charges not fully covered by your insurance will be billed to you after insurance processing is complete. You should contact your insurance company prior to your visit to check coverage and benefits. We accept a large number of insurance policies and it is impossible for the office staff to know exactly what is covered by your plan.


Cosmetic Procedures

For cosmetic procedures, payment is due at the time of service. We accept all major credit cards, cash, Care Credit and personal checks.

For your added assistance, we offer interest free “Care Credit” financing. It is easily attained and can be used for any type of health care as a line of credit. Their website,, is very informative and can calculate payments for you. They offer 0% interest for up to 12 months.


Insurance Forms

Insurance forms and family leave forms:  We are happy to help with these necessities of life, but will charge $25 to complete due to the extra time it takes for the doctors and staff to research and complete them.  We ask that under normal circumstances you give us at least two weeks to complete and mail any such form.

Service Fee: Effective February 25, 2013 – Due to the increasing number of Insurance Forms we are completing for patients, outside of office visits, we will be charging $25 for this service. This includes: FMLA Forms, Disability Forms, Life Insurance and any other Insurance Forms. We regret we can no longer provide this service for free.
